Group Health Insurance in Your CTC: Pricing What You Get

A group health premium in your CTC is worth to you what you would otherwise pay for similar cover, not the premium the employer quotes. To price it, compare three figures: the premium in the CTC, the after-tax value of the same sum paid to you as cash, and a real quote for a comparable policy. In the hypothetical below, a ₹15,000 premium is worth more than ₹15,000 of cash only if a comparable policy would cost you more than ₹15,000. At a 31.2% marginal tax rate, the break-even quote is ₹10,320.
Two offers with the same CTC
Both offers total ₹10,11,000 CTC. Offer A includes group health cover. Offer B pays the same sum in fixed pay instead.
| Component | Offer A | Offer B |
|---|---|---|
| Fixed pay | ₹9,60,000 | ₹9,75,000 |
| Employer PF (12% of ₹25,000 ceiling) | ₹36,000 | ₹36,000 |
| Group health premium | ₹15,000 | – |
| CTC | ₹10,11,000 | ₹10,11,000 |
EPFO's wage ceiling FAQ notes that CTC is not a statutory concept. It is the employer's presentation of cost. Two offers with the same CTC can therefore be worth different amounts to you.
Step 1: what the extra cash is worth after tax
In tax year 2026-27, Offer B's taxable income is ₹9,75,000 less the ₹75,000 standard deduction, or ₹9,00,000. That is within the new-regime rebate, so no tax is due on either offer. The extra ₹15,000 of cash is worth the full ₹15,000.
Now scale both offers up. Suppose Offer B's fixed pay is ₹30,15,000 and Offer A's is ₹30,00,000. Every extra rupee is then taxed in the 30% slab plus 4% cess, which is 31.2%. The extra ₹15,000 is then worth ₹10,320 in hand. The slabs are on the e-filing portal's rates page.
Step 2: price comparable cover yourself
Get a quote for a policy with the same members, sum insured, room-rent terms and waiting-period position as the group policy. Waiting periods for pre-existing conditions can differ between the two, so read both documents rather than assuming they match. Suppose your quote is ₹22,000.
| Case | Cash value of B's extra ₹15,000 | Your quote | Better for you |
|---|---|---|---|
| Rebate range | ₹15,000 | ₹22,000 | A, by ₹7,000 |
| 30% slab | ₹10,320 | ₹22,000 | A, by ₹11,680 |
| 30% slab, cheaper quote | ₹10,320 | ₹9,000 | B, by ₹1,320 |
The break-even quote equals the after-tax cash value. Above it, the group cover is worth more to you than the cash. Below it, the cash is.

print(compare(15000, 9000, 0.30 * 1.04))Step 3: adjust for deductions the group plan takes from you
Some group plans let you add parents for a premium deducted from your salary. That deduction reduces your in-hand; it is not part of the employer's ₹15,000. Suppose the parents' top-up costs ₹9,000 a year through payroll. Your monthly in-hand falls by ₹750. Compare it with a separate policy for your parents, and treat it as a separate decision from the main cover.
| Item | Paid by | Effect on in-hand |
|---|---|---|
| Employee and family cover | employer, inside CTC | none |
| Parents' top-up | you, through payroll | −₹750 a month |
Your payslip should show the top-up as a named deduction. If it appears without your written choice, ask payroll; payslip anatomy covers checking each deduction against what you agreed.
What the premium figure does not tell you
The ₹15,000 is the employer's cost, often for a pooled policy priced across many employees. It says little about how good the cover is for your family. The terms that decide real value are:
- sum insured, and whether it is shared across the family;
- room-rent or treatment sub-limits;
- co-payment percentage;
- waiting periods for pre-existing conditions and maternity;
- what happens to the cover when you leave.
A high premium with tight sub-limits can be worth less than a lower one without them.
Putting it in an offer comparison
List group health as a separate line with your own value beside it, not the employer's figure. For CTC lines that never reach you as cash, see CTC items you never receive as cash. Then compare offers on fixed pay plus the values you assigned.
Assigning a value to each line and stating the assumption behind it is ordinary analytical practice. Our Data Analyst programme runs 3–4 months, live online with optional in-person labs at our Noida centre.
Frequently asked questions
How is the employer-paid premium treated for my income tax?
We have not verified the treatment under the Income-tax Act, 2025 for this article, so it is left out of the arithmetic. Check how your payslip and year-end salary certificate show it, and ask payroll if it appears as a taxable perquisite.
What happens to the cover when I leave?
That depends on the policy, not on tax law. Ask HR for the policy summary and check the section on leaving employment before you rely on the cover for a planned treatment.
Can I claim a deduction for a policy I buy myself?
Under the new regime, section 202(2) excludes Chapter VIII deductions other than sections 124(1), 124(2), 125(2) and 146, so any deduction for a premium you pay would matter only if you opt for the old regime. We have not verified that deduction's section number or limits.
